Parental Consents Form

Are you agreeable to your child taking part in school assembly?
Do you give permission for your child’s photograph to be used in local newspapers, school brochures, school leaflets, etc.?
My child’s image may be published on the school website.
My child’s work may be published on the school website.
My child’s image may be published on the school’s social media.
My child’s work may be published on the school’s social media.
From time to time your child will be involved in cooking activities, science tasting activities etc. Do you give permission for them to eat the items involved, dependant on food allergies, religious beliefs etc.?
Do you give permission for us to discuss your child with their preschool and with outside agencies such as educational specialists, school nurse, medical authorities etc.?
During your child’s time with us, it is possible that local visits will be made within walking distance of the school. The children would be supervised by their class teacher and other adults. Do you give your consent to this?
